Abstract

Integrated care is a model whose differential is the presence of a coordination of actions of health. Its goal is to improve the quality of life of a population, especially the elderly with chronic diseases. There is a many discussion about the results of this model in view of the different practices found. This paper proposes to demonstrate the results of an integrated care model, through a retrospective cohort, belonging to a health care provider in Brasil. The performance indicators used were the number of emergency trips and the number of hospitalizations. There was a reduction in both indicators, however, there is a need for improvements in the evaluation methodology to achieve more robust results.
